## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT ^PRIVACY DATA OMITTED^ STATED HE WAS FLYING AN INSTRUMENT APPROACH INTO FTW MEACHAM FIELD IN A BELL 412, N911BE HELICOPTER. APPROXIMATELY 20 MILES OUT, THE SIDE PASSENGER ENTRANCE DOOR CAME OPEN AND DEPARTED THE HELICOPTER, CAUSING SKIN DAMAGE TO THE EXTERIOR TO THE HELICOPTER. DOOR HAD NOT BEEN REPORTED FOUND BY ANYONE, AS OF SEPTEMBER 30, 2009. PILOT STATED HIS COMPANY, MED TRANS WAS DELIVERING THIS HELICOPTER TO TEXAS AVIATION CENTER AT MEACHAM AIRPORT, AS IT WAS SOLD.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
