## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON NOVEMBER 5, 2009, AT APPROXIMATELY 12:55 LOCAL TIME, A ROBINSON HELICOPTER MODEL R-44 WITH REGISTRATION NUMBER N440M, PILOTED BY COMMERCIAL PILOT ISBELIA REYES DEPARTED FROM FERNANDO LUIS RIBAS DOMINICI AIRPORT (SIG) ON A 14 CFR PART 91. AROUND 13:10 LOCAL TIME THE HELICOPTER LANDED AT A PRIVATE PROPERTY IN VEGA ALTA, PR. AT THIS LOCATION MRS. REYES PICKED UP HER FATHER ^PRIVACY DATA OMITTED^, AND TWO FRIENDS ^PRIVACY DATA OMITTED^ AND ^PRIVACY DATA OMITTED^ TO CONTINUE TO MOROVIS, PR. AT APPROXIMATELY 13:20 LOCAL TIME MRS. REYES APPROACHED THE AREA INTENDED FOR LANDING, A HELIPAD 16 FEET WIDE BY 30.5 FEET LONG LOCATED AT AN ALTITUDE OF 1,391 FEET ABOVE SEA LEVEL, BEHIND A RESTAURANT IN MOROVIS, PR. AFTER THE FIRST RECOGNITION PASS, MRS. REYES BEGAN A CONFINED PINNACLE APPROACH. WHEN AT SHORT FINAL MRS. REYES DECIDED TO "GO AROUND" APPLYING THE COLLECTIVE BUT THE HELICOPTER DID NOT RESPOND ACCORDINGLY. THE LOW RPM BUZZARD AND LIGHT WAS ACTIVATED. WITH NO AIRSPEED AND NO ALTITUDE, MRS. REYES DECIDED TO PERFORM AN EMERGENCY LANDING. DUE TO THE UNRESPONSIVENESS OF THE HELICOPTER, MRS. REYES WAS NOT ABLE TO POSITION BOTH SKIDS ON TOP OF THE HELIPAD, LEAVING A PORTION OF THE RIGHT SKID OUT OF THE HELIPAD. SUDDENLY THE HELICOPTER FLIPPED OVER AND RESTED ON THE LEFT SIDE OF THE HELICOPTER.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA10CA051](https://openairdocs.org/ntsb/ERA10CA051), Nov 5, 2009: Robinson Helicopter R44 (N440M), Morovis, PR, substantial damage. Probable cause: The pilot’s delayed attempt to go around.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
