## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

N56489 DEPARTED ADDISON MUNICIPAL AIRPORT (ADS) AT 1555 LCL WITH PILOT ^PRIVACY DATA OMITTED^ AS PIC. ^PRIVACY DATA OMITTED^ PROCEEDED TO LAKE LEWISVILLE VIA DIRECT, AND CONFIGURED THE AIRCRAFT FOR A "GLASSY WATER" LANDING TO THE SOUTH. WITNESSES OBSERVED THE AIRCRAFT ON A FINAL APPROACH WITH THE GEAR(WHEELS) EXTENDED. UPON TOUCHDOWN, THE AIRCRAFT IMMEDIATELY FLIPPED ABOUT IT'S LATERAL AXIS, AND CAME TO REST INVERTED. THE PILOT WAS RECOVERED BY BOATERS, CPR INITIATED UNTIL REVIVED. HE COULD NOT RECALL HOW HE EXITED THE AIRCRAFT, DOORS WERE STILL SECURED, BUT WINDSCREEN WAS COMPLETELY REMOVED FROM THE AIRCRAFT ON IMPACT. PASSENGER WAS STILL IN THE AIRCRAFT, SEATBELT FASTENED. PASSENGER WAS THE ONLY FATALITY OF THE ACCIDENT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN10FA164](https://openairdocs.org/ntsb/CEN10FA164), Mar 18, 2010: Maule M-5-235C (N56489), Lewisville, TX, substantial damage, 1 fatal. Probable cause: The pilot did not retract the landing gear before a water landing. Contributing to the accident was the pilot did not use a checklist.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
