## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON 12/18/2010 WILLIAM JOHN FALLON WAS PIC OF N1462M, A CE 182-P, OWNED BY LAST WISH LLC, DURING A FLIGHT FOR THE PURPOSE OF DROPPING CREMATED HUMAN REMAINS. MR. FALLON ADVERTISED THIS SERVICE ON LASTWISH.COM AND WAS ASSISTED IN THIS OPERATION BY ^PRIVACY DATA OMITTED^. N1462M HAD BEEN STORED OUTSIDE; 4N1 AIRPORT CONDITIONS AT 1015 EST WERE -2° C WITH 62% RELATIVE HUMIDITY. AT 0945 EST A PROPANE FIRED ENGINE PRE-HEATER WAS INSERTED INTO THE ENGINE COWLING UPPER AIR INTAKES. ENGINE PRE-HEATING CEASED AT 1000 EST. ^PRIVACY DATA OMITTED^ STATED THAT SHORTLY AFTER TAKEOFF ON RUNWAY 24, THE ENGINE SEEMED TO LOOSE POWER AND MR. FALLON BEGAN A TURN AND TOLD ^PRIVACY DATA OMITTED^ TO BRACE HIMSELF. MARKS IN THE GRASS AND ON THE RUNWAY INDICATE THAT N1462M FIRST IMPACTED THE GRASS WITH THE PORT WING, APPROX. 5' WEST OF THE PAVED PORTION OF RUNWAY 24, NEAR IT'S FAR SOUTHERN END. SUBSEQUENT MARKS INDICATE THE AIRCRAFT'S NOSE IMPACTED THE GRASS AND THEN THE AIRCRAFT ROTATED APPROX. 180° AND CAME TO REST UPRIGHT. ^PRIVACY DATA OMITTED^, WHO HAD BEEN SEATED IN THE AFT, STARBOARD SEAT, EXITED THE AIRCRAFT AND CALLED THE POLICE AT 1022 EST. PRIOR TO THIS FLIGHT ON 12/10/2010 AN ANNUAL INSPECTION OF N1462M WAS LOGGED AND A TACHOMETER TIME OF 751.3 WAS NOTED. TACHOMETER TIME AT THE ACCIDENT SCENE WAS 751.4. TEMPERATURES FOR THE PERIOD BETWEEN 12/10/2010 AND 12/18/2010 WERE CONSISTENTLY BELOW FREEZING, WHILE N1462M WAS STORED OUTSIDE. POST ACCIDENT INVESTIGATION INDICATED SUFFICIENT FUEL AND MECHANICAL CONTINUITY OF THE ENGINE AND FLIGHT CONTROLS. THROTTLE, PROPELLER AND MIXTURE CONTROLS WERE ALL FULL-FORWARD AND THE CARBURETOR HEAT CONTROL WAS IN THE OFF / FORWARD POSITION. THE SEAL BELOW THE PORT FUEL CAP WAS LEAKING FUEL AT THE SCENE. ADDITIONALLY, ICE MIXED WITH CONTAMINANTS WAS FOUND WITHIN THE FUEL STRAINER BOWL. CONDITION OF THE PROPELLER INDICATED THAT THE ENGINE WAS NOT MAKING POWER UPON IMPACT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA11LA093](https://openairdocs.org/ntsb/ERA11LA093), Dec 18, 2010: Cessna 182 (N1462M), West Milford, NJ, substantial damage. Probable cause: The pilot's delayed application of carburetor heat while taking off in weather conditions conducive to serious carburetor icing, which resulted in a partial loss of engine power.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
