## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

IT WAS DETERMINED THE AIRCRAFT ENTERED A DYNAMIC ROLL OVER AFTER HARD CONTACT WITH THE LANDING SURFACE OCCURRED. IT IS BELIEVED, THE AIRCRAFT WAS SUBJECTED TO AN UNANTICIPATED GUST OF TAIL WIND CAUSING LOSS OF TAIL ROTOR EFFECTIVENESS (LTE). IT WAS FURTHER DETERMINED THAT NEITHER A FLIGHT CONTROL CONTINUITY NOR AIRWORTHINESS CHECK WAS CONDUCTED LEAVING AN OPEN QUESTION OF POSSIBLE MITIGATING AIRWORTHINESS ISSUES. DUE TO THE FINDINGS OF THIS SECONDARY INVESTIGATION IT WAS DECIDED TO CHANGE THE FAA RECOMMENDATION TO AIRMAN COUNSELING VS A 44709. THIS NOW COMPLETED AIRMAN COUNSELING INCLUDED METHODS FOR WIND DIRECTION DETERMINATION VIA PILOT OBSERVATION TECHNIQUES DURING ALL PHASES OF FLIGHT. WHICH WHEN COVERED, THE AIRMAN STATED THESE TECHNIQUES WERE NEW TO HIM AND HE WAS MOST APPRECIATIVE OF THE COUNSELING SESSION.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN11CA231](https://openairdocs.org/ntsb/CEN11CA231), Mar 17, 2011: Hiller UH-12D (N11CU), Iowa, LA, substantial damage. Probable cause: The pilot's inadequate compensation for gusty crosswinds during takeoff, which resulted in a loss of directional control.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
