## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JUNE 1, 2011 AT 1433 MDT, N475ER, A CESSNA 172R, WAS INVOLVED IN AN ACCIDENT WHEN THE PILOT WAS PERFORMING A GO-AROUND AFTER A LANDING ATTEMPT ON RUNWAY 26 AT WENDOVER, UT (ENV). THE WINDS AT THE TIME WERE 160@25G30. THE FLIGHT ORIGINATED FROM ST. GEORGE, UT (DXZ) SHORTLY AFTER 1200 MDT AND ENDED IN WENDOVER, UT (ENV) AT 1433 MDT. WITNESSES AT THE WENDOVER AIRPORT REPORTED HEARING THE PILOT STATE OVER THE RADIO THAT HE WAS LANDING RUNWAY 26 AND THAT HE WOULD NEED FUEL. THEY ALSO STATED THE PILOT HAD DIFFICULTY KEEPING THE AIRCRAFT ALIGNED WITH THE RUNWAY DURING THE LANDING SO ATTEMPTED TO GO-AROUND. WHEN THE AIRCRAFT WAS AT AN APPROXIMATE ALTITUDE OF 150-200 FEET, IT TURNED TO THE RIGHT, PITCHED DOWN, AND DESCENDED VERTICALLY TO THE GROUND. THE AIRCRAFT WAS DESTROYED AND THE PILOT AND THREE PASSENGERS WERE FATALLY INJURED. THE PLANNED DESTINATION FOR THE FLIGHT WAS UNCLEAR DURING THE INVESTIGATION DUE TO CONFLICTING INFORMATION. IT MAY HAVE BEEN PROVO, UT (PVU) OR WENDOVER, UT (ENV). THE PILOT REQUESTED ASSISTANCE FROM AIR TRAFFIC CONTROL AFTER BECOMING DISORIENTED AND WAS LOCATED 25 MILES SOUTH OF WENDOVER, UT IN A RESTRICTED AREA SO WAS DIRECTED TO WENDOVER, UT.
