## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT AND OBSERVER WERE CONDUCTING POWERLINE PATROL AT 150 FOOT AGL. THE PILOT HEARD AND FELT A LOUD BANG, FOLLOWED BY HIGH VIBRATIONS AND TOTAL LOSS OF TAIL ROTOR CONTROL. DURING THE EMERGENCY LANDING, THE LEFT SKID HIT THE GROUND FIRST, CAUSING THE HELICOPTER TO TIP OVER ON ITS SIDE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN11LA414](https://openairdocs.org/ntsb/CEN11LA414), Jun 23, 2011: Enstrom F-28C-2 (N5694A), Blair, NE, substantial damage. Probable cause: The failure of a tail rotor pitch change link due to the retaining bolt becoming separated in flight. Contributing to the accident was the pilot’s inadequate preflight inspection.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
