## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

^PRIVACY DATA OMITTED^ WAS ACTING AS A FLIGHT INSTRUCTOR FOR A STUDENT RECEIVING COMMERCIAL HELICOPTER FLIGHT TRAINING. THE STUDENT WAS PERFORMING A SIMULATED AUTOROTATION WITH A 180 DEGREE TURN AND LOST DIRECTIONAL CONTROL OF THE AIRCRAFT. ^PRIVACY DATA OMITTED^ DID NOT REESTABLISH CONTROL, AND WITH THE HELICOPTER TURNING QUICKLY TO THE LEFT IN CLOSE PROXIMITY TO THE GROUND HE ELECTED TO PUSH THE COLLECTIVE DOWN IN ORDER TO HAVE THE HELICOPTER LAND ON THE RUNWAY. THE HELICOPTER LANDED HARD HITTING SKIDS FIRST, THEN SLIDING BACKWARDS A SHORT DISTANCE AND TIPPED BACK ONTO THE TAIL BEFORE IT CAME TO A STOP.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN12CA009](https://openairdocs.org/ntsb/CEN12CA009), Oct 4, 2011: Robinson Helicopter R22 Beta (N4170M), Hibbing, MN, substantial damage. Probable cause: The private pilot's failure to maintain control of the helicopter during the landing flare.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
