## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THIS WAS A TANDEM PARACHUTE SKYDIVING INCIDENT INVOLVING TWO FATALITIES. THE INVESTIGATION WAS DONE BY THE LAS VEGAS FSDO. WITH THE ASSISTANCE OF A SKYDIVING SUBJECT MATTER EXPERT, SIMON WADE, IT WAS DETERMINED THE MAIN PARACHUTE FAILED TO DEPLOY AND OPEN PROPERLY.IT IS POSSIBLE THE TANDEM INSTRUCTOR MAY THEN HAVE FAILED TO PROPERLY USE THE RESERVE PARACHUTE SYSTEM. THIS CAN NOT BE VERIFIED. THE RESERVE PARACHUTE WRAPPED AROUND THE MALFUNCTIONING MAIN PARACHUTE. BOTH THE INSTRUCTOR AND STUDENT DIED AS A RESULT OF GROUND CONTACT.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
