## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AIRPLANE WAS CLEARED FOR TAKEOFF ON RWY 15. FROM TAXIWAY "A", PILOT TAXIED INTO TAKEOFF POSITION, APPLIED FULL POWER, AND RELEASED BRAKES. AS THE NOSE DIPPED AND THE AIRPLANE BEGAN TO ROLL FORWARD, A TAXIWAY LIGHT ASSEMBLY IMPACTED THE RIGHT PROPELLER, DAMAGING ALL THREE PROP BLADES AND THE RIGHT SIDE OF AIRCRAFT NOSE. THE INCIDENT SITE WAS BETWEEN TAXIWAYS "A" AND "B", ON RUNWAY CENTERLINE. THE LIGHT ASSEMBLY HAD BECOME DISLODGED FROM IT'S ORIGINAL POSITION ON THE PAVED NON-USE AREA AT THE APPPPROACH END OF THE RUNWAY. THE ORIGINAL LOCATION IS ON A POST LOCATED AT THE INTERSECTION OF THE EXTENDED CENTERLINE OF THE RUNWAY AND THE EXTENDED CENTERLINE OF THE TAXIWAY, AND STANDS APPROXIMATELY 24" TALL. IT IS NOT KNOWN HOW THE LIGHT ASSEMBLY BECAME DISLODGED FROM THE POST AND RELOCATED TO A POSITION APPROXIMATELY 150 FEET DOWN THE RUNWAY.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
