ON 8 NOVEMBER 2011, AT APPROXIMATELY 1100 HRS MST, DURING CLIMB OUT FROM CHEYENNE (CYS) AIRPORT, WY, N39EH, A PIPER MODEL PA-31T CHEYENNE AIRCRAFT SUSTAINED SUBSTANTIAL DAMAGE WHEN THE CABIN ENTRY DOOR DEPARTED THE AIRCRAFT AT APPROXIMATELY 8500 FEET MEAN SEA LEVEL (MSL). THE AIRCRAFT RETURNED TO CHEYENNE AIRPORT AND MADE AN UNEVENTFUL LANDING. THERE WERE NO INJURIES TO THE FOUR OCCUPANTS ON BOARD. WEATHER AT THE TIME WAS VISUAL METEOROLOGICAL CONDITIONS (VMC).

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR12LA030](https://openairdocs.org/ntsb/WPR12LA030), Nov 8, 2011: Piper PA-31T (N39EH), Cheyenne, WY, substantial damage. Probable cause: The flight crew’s failure to properly latch the cabin door before takeoff.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
