## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE PIC STATED HE HAD BEEN HAVING PROBLEMS WITH THE LOADS AS THEY WERE NOT PUT TOGETHER WELL BY THE "PICKERS". JUST BEFORE THE ACCIDENT THE PIC NOTICED THE LOAD WAS LOSE AND TRIED TO RELOCATE IT TO THE PICKER, WHO HAD ALREADY MOVED TO NEXT LOAD ABOUT 50 YARDS AWAY, AND HAVE HIM FIX THE LOAD. WHEN THE PIC STARTED TO MOVE THE LOAD, IT GOT CAUGHT ON SOMETHING ON THE GROUND AND CAUSED THE PIC TO LOSE CONTROL OF THE AIRCRAFT HITTING THE GROUND NOSE FIRST.THIS REPORT WILL BE AMENDED WHEN THIS OFFICE RECEIVES ADDITIONAL INFORMATION FROM THE PIC. THE PIC HAS LEFT THE COUNTRY TO NEW ZEALAND.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR12LA036](https://openairdocs.org/ntsb/WPR12LA036), Nov 14, 2011: Bell 206B (N16HA), Woodburn, OR, substantial damage. Probable cause: The pilot’s failure to maintain ground clearance while maneuvering with an external load line.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
