## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT ^PRIVACY DATA OMITTED^ WAS DEMOSTRATING A STRAIGHT IN AUTOROTATION IN A CLEAR GRASSY FIELD. PILOT DID NOT MAINTAIN PROPER CONTROL OF THE AIRCRAFT RESULTING IN THE AIRCRAFT CONTACTING THE GROUND. ONE OF THE TWO MAIN ROTOR BLADES STRUCK BOTH LEFT AND RIGHT HORIZONTAL STABILIZERS AND THE TAIL ROTOR DRIVE SHAFT COVER. A 44709 HAS BEEN OPENED BY FLM AND A LETTER SENT TO THE PILOT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA12CA095](https://openairdocs.org/ntsb/ERA12CA095), Dec 1, 2011: Bell 206L-3 (N216TV), Wingate, NC, substantial damage. Probable cause: The certified flight instructor did not select an adequate landing site for the practice autorotation.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
