## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON APRIL 1, 2012, AT APPROXIMATELY 1600 CDT, N9448Q DEPARTED FROM WOOSLEY FIELD (96KY), AN 1800 FOOT PRIVATE GRASS AIRPORT IN CALHOUN, KY. SHORTLY AFTER TAKEOFF, THE BEECHCRAFT BE-58 IMPACTED TERRAIN APPROXIMATELY 1600 FEET SOUTH OF THE AIRFIELD. THE AIRCRAFT WAS DESTROYED AND THE SOLE OCCUPANT OF THE AIRCRAFT SUFFERED FATAL INJURIES. WITNESSES REPORTED THAT SHORTLY AFTER THE AIRCRAFT LIFTED OFF FROM THE RUNWAY, IT BEGAN A LEFT HAND TURN AND CROSSED OVER THE TREES ADJACENT TO THE RUNWAY. ACCORDING TO THE WITNESSES, THIS TURN WAS UNCHARACTERISTIC FOR THE PILOT WHO NORMALLY WOULD FLY THE LENGTH OF THE RUNWAY, THEN RETURN AND OVERFLY THE FIELD, PRIOR TO TURNING ON COURSE. BASED ON THE DAMAGE TO THE AIRCRAFT'S ENGINES AND PROPELLERS, IT IS LIKELY THAT THE RIGHT ENGINE WAS PRODUCING POWER AT THE TIME OF IMPACT AND THAT THE LEFT ENGINE WAS NOT. THE LANDING GEAR WAS EXTENDED AT THE TIME OF IMPACT AND THE FLAPS WERE SET IN THE APPROACH POSITION. INVESTIGATION REVEALED THE PRESENCE OF FUEL THAT WAS CONTAMINATED WITH WATER IN ALL FUEL SYSTEM COMPONENTS FOR BOTH THE LEFT AND RIGHT ENGINES.

## The NTSB's investigation of this event (1)
