## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JUNE 26, 2012, UH1F HELICOPTER, N4582D, WAS INVOLVED IN AN ACCIDENT INJURING A GROUND WORKER WHILE CONDUCTING EXTERNAL LOAD OPERATIONS IN CLOSE PROXIMITY TO HIGH VOLTAGE TRANSMISSION LINES. N4582D WAS POSITIONING A CONCRETE BUCKET OVER A HIGH TENSION POWER LINE TOWER FOOT PAD WHILE CONCRETE WAS BEING POURED. DURING THE POUR, AN ELECTRICAL CHARGE FROM THE ADJACENT TRANSMISSION LINE ARCED TO THE SYNTHETHIC LONG LINE AND TRAVELED DOWN THE REMOTE HOOK ELECTRICAL LEAD RELEASING THE LOAD AND ENERGIZING THE BUCKET. A GROUND WORKMAN POURING CONCRETE COMPLETED THE CIRCUIT AND SUFFERED SEVERE ELECTRICAL BURNS REQUIRING HOSPITALIZATION.THERE WAS NO INJURY TO THE PILOT OR DAMAGE TO THE HELICOPTER.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR12LA279](https://openairdocs.org/ntsb/WPR12LA279), Jun 26, 2012: Bell UH-1F (N4582D), Concrete, WA, no damage. Probable cause: Arcing of an energized electrical power line to the external long line and the conduct of electrical energy to ground personnel.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
