## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

N416NV, A MD-83 WAS DEPARTING FROM DES MOINES INTERNATIONAL AIRPORT RUNWAY 23 AS ALLEGIANT AIR (WX0A) FLIGHT 497 AT OR AROUND 20:05 LOCAL TIME WITH 150 PASSENGERS, FOUR FLIGHT ATTENDANTS AND TWO PILOTS. DURING THE TAKEOFF ROLL, PRIOR TO ROTATION, THE LEFT ENGINE FAILED. ACCORDING TO THE ALLEGIANT GATE MANAGER IN DES MOINES, PASSENGERS REPORTED SEEING FIRE SHOOTING FROM THE LEFT ENGINE INLET. THERE ARE REPORTS THAT THE GRASS ALONG THE SIDE OF THE RUNWAY CAUGHT FIRE. THE ENGINE AND GRASS FIRE WAS CONTAINED. THE AIRCRAFT RETURNED TO THE GATE AND THE PASSENGER DEPLANED NORMALLY. THERE WERE NO INJURIES TO PASSENGERS OR CREW.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
