## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT HAD LEFT MAINTENANCE FACILITY IN BROUSSARD, LA ENROUTE BACK TO BEAUMONT, TX. APPROXIMATELY 25 MINUTES AFTER DEPARTURE, THE HELICOPTER STRUCK THE GUIDE WIRE OF A TOWER APPROXIMATELY 18 MILES SOUTH OF CROWLEY, LA. EYEWITNESS WAS A TOWER WORKER WHO WAS ON THE TOWER WORKING APPROXIMATELY 250 FT. UP THE TOWER AND SAW THE HELICOTER HEADING STRAIGHT FOR THE TOWER. WITNESS STATED THAT AT THE LAST MINUTE, THE PILOT SAW THE TOWER AND PITCHED UP AND LEFT, BUT STRUCK ONE OF THE GUIDE WIRES COMING OFF THE TOWER. THE GUIDE WIRE CUT THROUGH THE HELICOPTER'S NOSE SECTION, MAIN ROTOR SECTION, AND THE TAIL SECTION. THE MAIN ROTOR BLADES DEPARTED THE HELICOPTER ALONG WITH THE ENTIRE TAIL ROTOR SECTION AND CAME TO REST APPROXIMATELY 300 YARDS BEYOND THE TOWER.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN13FA009](https://openairdocs.org/ntsb/CEN13FA009), Oct 10, 2012: Bell 206B (N333DR), Crowley, LA, substantial damage, 1 fatal. Probable cause: The pilot’s failure to maintain clearance from the broadcast tower guy-wires.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
