## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

WHILE BEING MARSHALLED INTO THE "E" DEICE PAD, FDX1480 STRUCK A DEICE TRUCK AT THE #8 SLAT. THE NTSB CLASSIFIED THE EVENT AS AN ACCIDENT DUE TO THE EXTENSIVE WING DAMAGE. AFTER INVESTIGATION, FDEA FOUND THE CAUSE OF THE ACCIDENT TO BE SUPERVISORY ERROR BECAUSE THE DE-ICE CREW WAS NOT INFORMED THAT THE "E" DEICE PAD REQUIRED AN AIRCRAFT TO BE OFFSET FROM THE TAXIWAY CENTERLINE. FDEA IMPLEMENTED IMMEDIATE CORRECTIONS TO ITS DEICE PROCEDURES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB DCA13CA035](https://openairdocs.org/ntsb/DCA13CA035), Dec 26, 2012: Mcdonnell Douglas MD-11F (N612FE), Memphis, TN, substantial damage. Probable cause: the operator's failure to provide appropriate training and documentation for ground personnel, leading the marshaller to inadvertently guide the airplane into the collision.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
