ON THURSDAY JANUARY 17, 2013 AT APPROXIMATELY 12:45 PM EASTERN STANDARD TIME A STUDENT PILOT AND FLIGHT INSTRUCTOR OPERATING A SINGLE ENGINE ROTORCRAFT ROBINSON MODEL R-22 BETA SERIAL NUMBER 2353 CRASH LANDED TO THE NORTH ON RUNWAY NUMBER 01 AT ELLINGTON AIRPORT, CT (7B9). THE PILOTS SUSTAINED MINOR INJURIES UPON IMPACT PERFORMING A STRAIGHT-IN PRACTICE AUTO ROTATION APPROACH TO THE HARD SURFACE OF THE RUNWAY. THE ROTORCRAFT CAME TO REST AT THE POINT OF IMPACT ON THE RIGHT SIDE OF THE FUSELAGE FACING 180 DEGREES TOWARDS THE SOUTH END OF THE RUNWAY WITH SUBSTANTIAL DAMAGE AND NO POST-ACCIDENT FIRE. UPON FURTHER INVESTIGATION OF THE ROTORCRAFT WRECKAGE, IT WAS NOTED THAT A BOLT FOR THE COLLECTIVE CONTROL SYSTEM PUSH-PULL TUBE ASSEMBLY THAT ATTACHES TO THE JACKSHAFT ASSEMBLY WAS FOUND DETACHED. THE BOLT AND SUBSEQUENT HARDWARE WERE FOUND LOCATED WITHIN THE ROTORMAST FAIRING STRUCTURE AND AT THE LOWER FUSELAGE ASSEMBLY BENEATH THE STRUCTURE FOR SUPPORT OF BOTH THE PILOT AND CO-PILOT SEATS.
