## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON 4-12-2013 @ 1612 LOCAL, MARICOPA SHERIFF (MCSO) PILOT ^PRIVACY DATA OMITTED^ WAS PRACTICING AUTO-ROTATIONS WITH MCSO - INSTRUCTOR ^PRIVACY DATA OMITTED^ IN N119FT A BELL OH-58C AT KDVT. ^PRIVACY DATA OMITTED^ WAS "SLOW" TO THROTTLE BACK UP AND SUBSEQUENTLY IMPACTED THE GROUND SLIDING FORWARD UNTIL TIPS OF THE SKIDS DUG INTO THE SOFT DIRT. THE SUDDEN FORWARD MOMENTUM ARCHED THE TAIL BOOM UPWARDS AND FLEXED THE MAIN ROTOR BLADES DOWNWARDS, AT THAT POINT THE MAIN BLADES CONTACTED THE TAIL BOOM CHOPPING OFF THE TAIL ROTOR. NO INJURIES TO THE CREW OF TWO. THE HELICOPTER'S FUSELAGE WAS ALSO DAMAGED BY THE IMPACT TO THE GROUND.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR13TA198](https://openairdocs.org/ntsb/WPR13TA198), Apr 12, 2013: Bell OH-58C (N119FT), Phoenix, AZ, substantial damage. Probable cause: The pilot receiving instruction's delay in adding power during a recovery from a practice autorotation.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
