## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ACCORDING TO THE PILOT, THIS LIFEGUARD FLIGHT ORIGINATED FROM WEST, TEXAS FOLLOWING A FERTILIZER PLANT EXPLOSION WITH MULTIPLE DEATHS AND INJURIES. THE PILOT SAW A BIRD ABOUT FOUR FEET FROM THE HELICOPTER CANOPY WHILE TRAVELING AT 120 KTS. THE BIRD HIT AND PENETRATED THE WINDSHIELD ON THE RIGHT SIDE, ABOUT THE LEVEL OF THE INSTRUMENT PANEL. THE BIRD STRUCK THE BACK OF THE INSTRUMENT PANEL, DENTING THE CASE, HIT THE PILOT?S ARM AND RIGHT DOOR FRAME AND SEVERAL AREAS INSIDE THE CABIN, FINALLY COMING TO REST AT THE PATIENT?S FEET.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
