## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JUNE 28, 2013, AT APPROXIMATELY 12:15 LOCAL TIME, THE REGIONAL OPERATIONS CENTER NOTIFIED THE TELEPHONE DUTY ASI OF A REPORTED ACCIDENT AT MILE POST 195 ADJACENT TO THE PARKS HIGHWAY NEAR CANTWELL, AK. ALASKA STATE TROOPERS ON SCENE CONFIRMED THREE FATALITIES.

THE AIRCRAFT, REGISTRATION IS N5JG, WHICH IS A BE-95-B55 (T42A). THE PIC OF THE AIRCRAFT WAS THE REGISTERED OWNER. THE PIC HELD AN ATP RATED PILOT WITH APPROXIMATELY 4900 FLIGHT HOURS. THERE WERE TWO PASSENGERS ON BOARD WHO WERE BOTH FATALLY INJURED. ONE OF THE PASSENGERS WAS A COMMERCIAL, INSTRUMENT, MULTI ENGINE RATED PILOT AND WAS BELIEVED TO HAVE BEEN IN THE RIGHT SEAT. THE FAA DATABASE INDICATES THIS PILOT HAD IN EXCESS OF 10,000 FLIGHT HOURS.

THE AIRCRAFT WAS PART OF A PART 91 MULTI AIRPLANE GUIDED MEMBERSHIP OPERATION. THE GROUP DEPARTED FAIRBANKS (FAI) FRIDAY, JUNE 28 AT APPROXIMATELY 10:00 AM, ENROUTE TO HOMER (HOM). THE GROUP, INCLUDING N5JG, RECEIVED A WEATHER BRIEFING AT 0600 LOCAL TIME FOR THE FLIGHT. N5JG WENT AHEAD OF THE OTHER AIRCRAFT TO SCOUT THE ROUTE OF FLIGHT. WEATHER CONDITIONS AT THE TIME OF THE ACCIDENT APPEARED TO BE LOW CEILINGS AND OBSCURATION FROM SMOKE AND LIGHT RAIN. THERE WERE SEVERAL FIRES IN THE AREA CONTRIBUTING TO THE SMOKEY CONDITION.

A LARGE DEBRIS FIELD, EXTENDING FROM SOUTH TO NORTH, WITH WHAT APPEARED TO BE A POST-CRASH FIRE. INITIAL INDICATIONS ARE THAT THE AIRCRAFT STRUCK THE GROUND IN A NEARLY LEVEL PITCH ATTITUDE, APPARENTLY LEFT WING LOW, INDICATING THE AIRCRAFT MAY HAVE BEEN IN A LEFT TURN.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ANC13FA058](https://openairdocs.org/ntsb/ANC13FA058), Jun 28, 2013: Beech 95-B55 (N5JG), Cantwell, AK, destroyed, 3 fatal. Probable cause: The pilot’s continued visual flight into instrument meteorological conditions, which resulted in an in-flight collision with mountainous terrain.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
