## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JULY 29, 2013, AT APPROXIMATELY 0605 CDT, N10009 A TEXAS HELICOPTER OH-13H/M47A, IMPACTED THE GROUND IN A CORN FIELD NEAR ^PRIVACY DATA OMITTED^, LEONORE ILLINOIS DURING AERIAL APPLICATION. THE PILOT IN COMMAND ^PRIVACY DATA OMITTED^ WAS THE ONLY PERSON ON BOARD AND SUSTAINED A MINOR INJURY. VISUAL METEOROLOGICAL CONDITIONS PREVAILED AT THE TIME OF THE ACCIDENT. THERE WAS NO POST-CRASH FIRE. THE AIRCRAFT WAS TOTALLY DESTROYED.

UPON EXAMINATION OF THE MAIN BODY WRECKAGE, THE FUEL SELECTOR SWITCH WAS IN THE RIGHT TANK POSITION, AND THERE WAS CONSIDERABLE FUEL STILL AVAILABLE IN THE RIGHT TANK. OIL LEVEL COULD ONLY BE DETERMINED BY REMOVING THE OIL FILTER AND CHECKING FOR OIL. THE FILTER WAS FULL AND THE OIL APPEARED CLEAR, FREE OF CONSIDERABLE DEBRIS. CIRCUIT BREAKERS APPEARED IN THE NORMAL POSITION EXHIBITING NO SIGNS OF MALFUNCTION. THERE WAS NO SIGN OF FIRE OR BURNING TO CONTROL WIRING TO THE FUEL SYSTEM OR AIRCRAFT CONTROL WIRING. THE PILOT-IN COMMAND WAS INTERVIEWED AND IT WAS NOTED THAT HE EXPERIENCED NO MECHANICAL ABNORMALITIES PRIOR TO IMPACT.

## The NTSB's investigation of this event (1)
