## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON 9/3/2013 AMERIFLIGHT 2688 DEPARTED KPHX AND DURING CLIMB OUT THE "DUCT LEAK" LIGHT ILLUMINATED. THE FLIGHT CREW FOLLOWED THE ORH PROCEDURES AND RETURNED TO KPHX SAFELY.

THE AIRCRAFT ON 8/30/2013 EXPERIENCED A SIMILAR EVENT FOR A "DUCT LIGHT ILLUMINATING" DURING CLIMB OUT.

AMERFIFLIGHT DETERMINED THROUGH TROUBLESHOOTING THAT THE L/H PRESSURE REGULATOR VALVE P/N 38E96-5A IS FAILING INTERNALLY CAUSING THE UNRELIEVED PRESSURE TO BLOW THE RECEIVER OFF THE BOTTOM OF THE VALVE. THE RECEIVER IS HELD ON BY A SMALL ADJUSTABLE CLAMP.

THE AIR CARRIER HAS SENT THE SUSPECT VALVE OUT FOR REPAIR AND AN SDR & M OR D HAVE BEEN COMPLETED.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
