## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ALLEGIANT AIR LLC FLIGHT 436 OPERATING FROM LAS VEGAS, NEVADA TO PEORIA, ILLINOIS EVACUATED PASSENGERS ON RAMP IN FRONT OF THE AIRPORT FIRE DEPARTMENT. BOARDING WAS ACCOMPLISHED WITHOUT INCIDENT, THE SAFETY DEMO WAS PERFORMED AND THE FLIGHT ATTENDANTS WERE INSTRUCTED TO TAKE THEIR SEATS FOR TAKE-OFF. THE FLIGHT ATTENDANTS SEATED IN THE AREA OF THE LAVATORIES SAW THE LAVATORY INDICATORS FLASHING AND ALARMS SOUNDING. FA1 CALLED THE CAPTAIN AND INFORMED HIM OF THE ALARMS. THE CAPTAIN RESPONDED AND TOLD FA1 HE WOULD GET BACK TO HIM. FA1 ALERTED THE AFT FLIGHT ATTENDANTS AND THE ATTENDANTS CHECKED THE LAVATORIES PER THEIR FIRE FIGHTING PROCEDURES. THE CAPTAIN CALLED TO THE CABIN AND TOLD THEM THEY COULD NOT FIND ANYTHING WRONG BUT WERE RETURNING TO THE GATE SO THAT MAINTENANCE COULD INSPECT THE AIRCRAFT. TAXIING BACK TO THE GATE THE AFT FLIGHT ATTENDANT ALERTED FA1 THAT HE COULD SEE A MIST IN THE CABIN. FA2 CALLED THE CAPTAIN AND REPORTED THE HAZE. AT THIS TIME FA1 NOTICED THE FOG/SMOKE IN THE CABIN AND ALSO INFORMED THE CAPTAIN. THE CAPTAIN ADVISED TO EVACUATE THE AIRCRAFT USING THE FORWARD DOORS AND TAILCONE. HE TAXIED TO THE AREA NEAR THE FIRE STATION, STOPPED THE AIRCRAFT AND ISSUED THE EVACUATION. THE AFT FLIGHT ATTENDANT OPENED THE DOOR TO THE TAILCONE. THE TAILCONE DEPLOYED BUT THE SLIDE PACK DID NOT FALL AND DEPLOY THE SLIDE. THE FA PUSHED THE CONTAINER HOUSING THE TAILCONE SLIDE OUT AND THE SLIDE DEPLOYED AUTOMATICALLY. FA3 OPENED THE 1R AND STARTED HER EVACUATION. FA1 OPENED THE 1L BUT THE SLIDE DID NOT DEPLOY AUTOMATICALLY SO HE REACHED DOWN AND PULLED THE MANUAL INFLATION HANDLE. HE CALLED THE PASSENGERS TO HIS DOOR AND AS THE FIRST PERSON APPROACHED HE HEARD A POPPING SOUND AND LOOKED TO SEE THAT THE SLIDE DEFLATED. HE BLOCKED HIS EXIT AND REDIRECTED THE PASSENGERS TO 1R. ONCE ALL THE PASSENGERS HAD EVACUATED THE CREW NOTICED THAT THERE WAS NO SMOKE BUT THAT THE AREA BEHIND THE AFT LAVATORIES WERE HOT. THERE WAS NO UNUSUAL SMELL OR SMOKE. THE CREW EVACUATED, GATHERED THE PASSENGERS AND DID A HEAD COUNT OF 144+1. THE FIRE DEPARTMENT BOUGHT OUT WATER AND GATORADE AND THE EMTS WERE AVAILABLE TO ASSIST PASSENGERS. THERE WAS ONE SLIGHT INJURY TO A MAN THAT HAD A BURN FROM EVACUATING DOWN THE SLIDE. THE PASSNEGERS WERE BOARDED ON BUSES TO THE TERMINAL. A SEPARATE VAN TOOK THE CREW TO THE TERMINAL FOR THE DEBRIEF.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
