THE PILOT, THE SOLE OCCUPANT ON BOARD, WAS FATALLY INJURED, AND THE AIRCRAFT WAS DESTROYED BY THE IMPACT AND SUBSEQUENT FIRE. WITNESSES STATE THAT THEY SAW THE MAIN ROTOR HEAD DEPART THE AIRCRAFT IN-FLIGHT, IMMEDIATELY BEFORE IT IMPACTED THE GROUND.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN14LA056](https://openairdocs.org/ntsb/CEN14LA056), Nov 16, 2013: Pearson Jon M Dragonfly (N975DF), Anahuac, TX, destroyed, 1 fatal. Probable cause: The incorrect installation by the pilot, who was not a certificated mechanic or repairman, of the main rotor head bearing block, which resulted in the in-flight separation of the rotor head.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
