## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON APRIL 4, 2014 THE OPERATOR WAS CONDUCTING ANUAL RECURRENT TRAINING AND CHECKING AT THE MISSOULA INTERNATIONAL AIRPORT, MISSOULA, MONTANA. N43MA, A BELL 206 HELICOPTER WAS IN USE FOR THE FLIGHT TRAINING/CHECKING. ON THIS FLIGHT THE PILOT WAS CONDUCTING TRAINING/EVALUATION WITH PROSPECTIVE NEW HIRE. THE PROSPECTIVE NEW HIRE HAD MADE A NORMAL APPORACH TO A LANDING AT THE SITE SELECTED BY THE INSTRUCTOR PILOT. THE INSTRUCTOR PILOT WAS GOING TO DEMONSTRATE A POWER FAILURE FROM A HOVER, INITIATED THE LIFT-OFF WHEN THE AIRCRAFT EXPERIENCED DYNAMIC ROLLOVER AND ROLLED OVER ONTO IT'S RIGHT SIDE. TAKEOFF SURFACE WAS DIRT. NEW HIRE WAS NOT PILOT FLYING.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR14LA159](https://openairdocs.org/ntsb/WPR14LA159), Apr 4, 2014: Bell 206B III (N43MA), Missoula, MT, substantial damage. Probable cause: A dynamic rollover during attempted liftoff.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
