AFTER THE FORMATION BROKE UP THE JUMPER ACTIVATED HIS MAIN CANOPY AND WAS OBSERVED BY NEARBY JUMPERS TO BE DESCENDING UNDER A PARTIALLY INFLATED MAIN CANOPY WITH LINE TWISTS. OBSERVERS STATED THEY SAW THE CANOPY SPINNING AND THERE WAS NO ATTEMPT BY THE JUMPER TO REACT TO THE SPINNING CANOPY OR TO ATTEMPT ANY TYPE OF EMERGENCY PROCEDURES. HE WAS OBSERVED TO BE SLUMPED IN HIS HARNESS WITH HIS ARMS HANGING DOWN TO HIS SIDES AND HEAD DOWN THRU OUT THE DESCENT.

THE JUMPER IMPACTED THE EARTH WHILE STILL IN A SPIN AND WAS DETERMINED TO BE DECEASED BY FIRST RESPONDERS TO THE SCENE.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
