## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PARACHUTIST ^PRIVACY DATA OMITTED^ WAS PERFORMING HIS 6TH SKYDIVE WITH SKY DIVE SANTA BARBARA, OUT OF LOMPOC AIRPORT, LOMPOC, CALIFORNIA. INSPECTION OF THE PARACHUTE REVEALED NO DISCREPANCIES EXCEPT FOR THE LINES AND HARNESS THAT WERE CUT TO REMOVE THE INJURED PARACHUTIST. THE RESERVE PARACHUTE WAS DUE FOR REPACK ON 13 OCTOBER 2014.

INITIATED FORM 8020-23 IN ACCORDANCE WITH THE REQUIREMENT DEFINED IN FAA ORDER 8020.11C, PARAGRAPH 12, PAGES 5-9.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
