## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

DURING ROUTINE SURVEILLANCE, FAA INSPECTORS FROM THE DES MOINES FLIGHT STANDARDS DISTRICT OFFICE (DSM FSDO) DISCOVERED WRECKAGE FROM A DAMAGED HELICOPTER INSIDE A HANGAR. THE WRECKAGE WAS INSPECTED AND SEVERAL PHOTOGRAPHS WERE TAKEN BY FAA INSPECTORS. EMPLOYEES AT THE FACILITY INDICATED THE PARTS WERE FROM A HELICOPTER THAT CRASHED ON JUNE 10, 2014 IN HUDSON, WI, AND THAT THEY KNEW THE PILOT IN COMMAND. IT WAS ALSO REPORTED THAT THE PILOT HAD A PASSENGER WITH HIM AND THAT HE DID NOT HAVE A HELICOPTER RATING AT THE TIME OF ACCIDENT. STATEMENTS SHOWED THE CRASH OCCURRED SHORTLY AFTER TAKEOFF FROM A PARKING LOT ADJACENT TO AN OFFICE BUILDING IN THE CITY OF HUDSON, WI. STATEMENTS ALSO INDICATED THAT THE HELICOPTER LOST POWER AFTER TAKEOFF. FAA INSPECTORS HAVE BEEN UNABLE TO CONFIRM OR DISPROVE THIS.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN14LA537](https://openairdocs.org/ntsb/CEN14LA537), Jun 10, 2014: Hughes TH 55A (N5335U), Hudson, WI, substantial damage. Probable cause: The nonhelicopter-rated pilot’s operation of the helicopter and its subsequent collision with trees during takeoff for reasons that could not be determined based on the available evidence.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
