TENDEM PARACHUTE JUMP ORIGINATED FROM PENNRIDGE AIRPORT (CKZ). ALL JUMPERS EXITED THE AIRCRAFT NORMALLY AT 12,000 FEET. THE INSTRUCTOR DEPLOYED THE MAIN CANOPY AT A NORMAL ALTITUDE OF APPROXIMATELY 6,000 FEET. THE CANOPY MALFUNCTIONED CAUSING A VIOLENT SPIN. THE INSTRUCTORS ATTEMPT TO JETTISON THE MAIN CANOPY WAS UNSUCESSFUL RESULTING IN ONLY A PARTIAL CUTAWAY "STREAMER". THE INSTRUCTOR THEN DEPLOYED THE RESERVE CANOPY WHICH FOULED IN THE MAIN CANOPY. THE MAIN CANOPY WAS THEN SUCESSFULLY JETTISONNED. THERE WAS INSUFFICIENT ALTITUDE REMAINING FOR THE INSTRUCTOR TO INFLATE AND BRAKE THE RESERVE CANOPY WELL ENOUGH TO PREVENT SERIOUS INJURY TO THE INSTRUCTOR AND STUDENT/PASSENGER.

THE MAIN CANOPY WAS FOUND LOCATED AN UNUSUALLY SHORT DISTANCE FROM THE INCIDENT SITE.

THE MAIN CANOPY WAS FOUND TO HAVE CANOPY MATERIAL FROM THE RIGHT REAR CORNER PULLED APPROXOMATELY 20 INCHES THROUGH THE SLIDER GROMMET. NO OTHER ANOMOLIES WERE NOTED WITH THE MAIN CANPOPY, RESERVE CANOPY, OR HARNESS.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
