THE AIRMAN WAS INVOLVED IN A FATAL AIRCRAFT ACCIDENT WITH 1 PASSENGER ON BOARD FOR A TOTAL OF 2 FATALITIES. ATC TAPES INDICATE THAT THE AIRMAN WAS RECEIVING FLIGHT FOLLOWING AND WAS UNABLE TO MAINTAIN VFR CONDITIONS. CONTROLLER STATEMENTS INDICATE THAT DURING THE LAST CONTACT WITH THE PILOT, THE PILOT STATED HE WAS ENCOUNTERING IMC CONDITIONS. A CONTROLLER STATEMENT INDICATES THE AIRCRAFT WAS AT 7500 MSL. NO FURTHER TRANSMISSIONS WERE RECEIVED SO SEARCH AND RESCUE WAS INITIATED AS THE TRANSPONDER BEACON WAS NO LONGER VISIBLE TO THE CONTROLLER.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN14LA424](https://openairdocs.org/ntsb/CEN14LA424), Aug 9, 2014: Barger Steve M RV-7A (N92BF), Russell, IA, destroyed, 2 fatal. Probable cause: The noninstrument-rated pilot’s inadvertent entry into instrument meteorological conditions, which resulted in his spatial disorientation and the subsequent loss of airplane control.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
