## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE POI WAS INFORMED BY THE CHIEF PILOT ON THE MORNING OF 9-30-2014 THAT AN AIRCRAFT OPERATED BY GULA WAS INVOLVED IN A PROP STRIKE DURING START-UP. THE FLIGHT WAS A PART 91 REPOSITIONING FLIGHT. THE POI AND OTHER INSPECTORS WENT TO THE AIRPORT AND VIEWED THE AIRCRAFT AND RAMP VIDEO FOOTAGE THAT WAS GRAINY. THE VIDEO FOOTAGE SHOWED THE AIRCRAFT STARTING, IMMEDIATELY SHUTTING DOWN AND THEN THE CREW EXITING THE AIRCRAFT AND PROCEEDING TO THE RIGHT ENGINE. INVESTIGATION INVOLVED LITTLE TO NO DAMAGE TO THE PROPELLER AND ENGINE. DENTS WERE FOUND ON THE RIDE SIDE OF THE NOSE OF THE AIRCRAFT THAT MATCHED THE SIZE AND SHAPE OF A TRAFFIC CONE. VIDEO FOOTAGE SHOWED THE PIC MAKING A QUICK WALK-AROUND THE AIRCRAFT, BUT APPARENTLY MISSED THE TRAFFIC CONE IN THE RIGHT ENGINE PROPELLER ARC.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
