## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT ^PRIVACY DATA OMITTED^ SELECTED GEAR UP ON TAKE OFF FROM KNSA, SAW THREE GREEN INDICATOR LIGHTS WENT OUT, NO RED IN-TRANSIT LIGHTS, AND DECREASING AIRSPEED. HE CYCLED THE GEAR SEVERAL TIMES, BUT THE UNSAFE CONDITION REMAINED. ON APPROACH INTO BURBANK/BOB HOPE AIRPORT, ^PRIVACY DATA OMITTED^ PERFORMED THE EMERGENCY GEAR DOWN PROCEDURES. WHEN THE CIRCUIT BREAKERS WERE CYCLED HE NOTICED THE TWO MAIN GEAR GREEN LIGHTS FLICKERED, BUT NO NOSE GEAR LIGHT INDICATION. BURBANK TOWER CONFIRMED ALL GEAR WAS DOWN, AFTER THE PILOT MADE A FLY-BY. UPON LANDING THE NOSE GEAR COLLAPSED. THE AIRCRAFT SUSTAINED PROPELLER AND UNDERCARRIAGE DAMAGE.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
