## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AIRCRAFT WAS ON A VFR CROSS COUNTRY FLIGHT ORIGINATING IN MINNESOTA. NO FLIGHT PLAN INFORMATION EXISTS FOR THIS FLIGHT. FAMILY AND FRIENDS STATED THAT THIS PARTICULAR LEG OF THE FLIGHT WAS PLANNED TO BE FROM COULEE CITY, WA TO ORCAS ISLAND, WA. THE AIRCRAFT NEVER ARRIVED AT ITS PLANNED DESTINATION AT ORCAS ISLAND. AFTER OVERDUE CALLS WERE MADE ON 7-14-2015, A SEARCH FOUND THE AIRCRAFT WRECKAGE LOCATED IN STEEP TERRAIN ON TWIN SISTERS MOUNTAIN NEAR ACME, WA. AT APPROXIMATELY 6300 MSL AND APPROXIMATELY 146 MILES FROM ITS PLANNED DEPARTURE POINT. FURTHER INVESTIGATION AT THE ACCIDENT LOCATION BY RESCUE PERSONNEL INDICATED THAT THE TWO OCCUPANTS SUFFERED FATAL INJURIES AND THE AIRCRAFT WAS DESTROYED UPON IMPACT WITH TERRAIN.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR15FA215](https://openairdocs.org/ntsb/WPR15FA215), Jul 11, 2015: Piper PA 22-160 (N9712D), Twin Sisters Mtn, WA, destroyed, 2 fatal. Probable cause: The noninstrument-rated pilot's continued visual flight into instrument meteorological conditions, which resulted in controlled flight into mountainous terrain.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
