## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE AIRCRAFT WAS ENROUTE TO KOSH WITH THE NON-RATED PILOT^PRIVACY DATA OMITTED^, A CERTIFIED FLIGHT INSTRUCTOR, AND A PASSENGER ON BOARD. THE AIRCRAFT DEPARTED KUAO AND LANDED IN KLWS FOR FUEL. ^PRIVACY DATA OMITTED^ INSTRUCTED THE LINEMAN AT KLWS TO REFUEL THE AIRCRAFT TO THE BOTTOM OF THE SCREEN.

AFTER REFUELING, THE NON-RATED PILOT^PRIVACY DATA OMITTED^ AND THE CFI PERFORMED A WALK AROUND INSPECTION WHICH INCLUDED A VISUAL INSPECTION OF THE FUEL LEVEL. THE FUEL SUMPS WERE NOT DRAINED. THE ENGINES OPERATED NORMALLY AND ALL INSTRUMENT INDICATIONS WERE NORMAL DURING THE TAXI TO RUNWAY 12. THE AIRCRAFT STARTED THE TAKE-OFF ROLL WHEN, AT ROTATION, THE ENGINES LOST POWER.

THE CFI TOOK OVER THE CONTROLS AND APPLIED HEAVY BRAKING BRINGING THE AIRCRAFT TO A STOP ON THE REMAINING RUNWAY. THE AIRCRAFT TAXIED CLEAR OF THE RUNWAY AND WAS GIVEN CLEARANCE BACK TO THE RAMP.

THE LINEMAN MARSHALLED THE AIRCRAFT TO PARKING AND STATED HE MISTAKENLY FUELED THE AIRCRAFT WITH JET-A INSTEAD OF AVGAS.

THE FUEL RECEIPT SHOWED THE AIRCRAFT WAS FUELED WITH 95 GALLONS OF JET-A.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
