## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AT ABOUT 1:30 ON TUESDAY AUGUST 18, 2015, THE PILOT TOOK OFF WITH 25 GALLONS OF FUEL AND 80 GALLONS OF CHEMICAL TO SPRAY A BEAN FIELD 7 MILES EAST OF CRESCO, IA. ABOUT 5 MINUTES INTO THE SPRAY RUN AT ABOUT 5 FEET ABOVE THE FIELD THE ENGINE FAILED. THE PILOT STATED THAT HE NOTICED THE MAIN ROTOR RPM BLEEDING OFF AND EXECUTED AN AUTO ROTATION TO A WATER WAY ON THE SIDE OF A HILL. UPON TOUCH DOWN ON THE UNLEVEL TERRAIN, THE MAIN ROTOR CONTACTED THE TAILBOOM AND IT SEPARATED FROM THE AIRCRAFT. ONCE THE BLADES STOPPED SPINNING, THE PILOT SHUT THE FUEL VALVE OFF AND EXITED THE HELICOPTER. THE HELICOPTER WAS SITTING UP RIGHT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN15LA370](https://openairdocs.org/ntsb/CEN15LA370), Aug 18, 2015: Bell 206B (N5743W), Cresco, IA, substantial damage. Probable cause: The loss of engine power due to the high-cycle fatigue failure of the third-stage turbine wheel.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
