## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE MAIN PARACHUTE FAILED TO COMPLETELY OPEN. A WITNESS STATED THAT A LINE OVER THE CHUTE PREVENTED IT FROM PROPERLY OPENING. WHEN THE PARACHUTE JUMPER TRIED TO USE THE CUT AWAY DEVICE, THE RIGHT SIDE RISER CUT AWAY PROPERLY AND THE LEFT RISER REMAINED ATTACHED TO THE PACK. THE DEPLOYED BUT SPINNING CHUTE MATERIAL TWISTED THE LEFT RISER TO THE POINT THAT IT PREVENTED THE REMAINING CUTAWAY CABLE FROM OPERATING AT ALL. THE PARACUTE JUMPER THEN DEPLOYED HIS RESERVE CHUTE JUST ABOVE THE TREE TOP LEVEL BUT TOO LATE TO DEPLOY.

JUMPERS NAME:CRAWFORD, BRUCE DEWAYNE

EXPERIENCE AT JUMPING: UNKNOWN. A WITNESS STATED THAT HE KNEW BRUCE WAS AN EXPERIENCED JUMPER.

CLUB AFFILIATION: UNKNOWN

PARACHUTE WAS LAST INSPECTED ON: 09-09-2015 BY 3639796QEY

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
