## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AIRCRAFT WAS BEING FLOWN AS A TEST FLIGHT AFTER COMPLETION OF A ANNUAL INSPECTION. SLIGHTLY BEFORE ROTATION SPEED (APROX 80KTS) THE NOSE GEAR COLLAPSED CASING MINOR DAMAGE TO THE UNDERBELLY AND PROP STRIKES ON BOTH ENGINES. A DEPARTING PIECE OF PROP ALSO DAMAGED THE LEADING EDGE OF THE RIGHT WING HALF WAY DOWN THE WING.

AIRCRAFT ANNUAL INSPECTION WAS NOT ENDORSED PRIOR TO THE TEST FLIGHT ACTION. THE AIRCRAFT HAD BEEN ISSUED A NEW N# SEVERAL YEARS AGO AND THE AIRWORTHINESS CERT WAS NEVER UPDATED.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
