## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON 22 MARCH 2016, AN ADVENTURA II, N709CW, SUFFERED AN INFLIGHT ENGINE FAILURE ON TAKE-OFF, DEPARTING ROCKLEDGE AIRPARK, RUNWAY 36 TO ROCKLEDGE AIRPARK HEADING NORTH AS A MAINTENANCE FLIGHT. THIS INCIDENT OCCURRED AT 1355. I WAS NOTIFIED AT 1445. THE AIRCRAFT LANDED ON GUS HIPP BLVD, ROCKLEDGE, FL, JUST NORTH OF THE AIRPORT. THERE WAS ONE PERSON ON-BOARD, THE PILOT, ^PRIVACY DATA OMITTED^. THERE WERE NO INJURIES AND NO DAMAGE TO THE AIRCRAFT.

INITIAL CONTACT WITH THE ROCKLEDGE POLICE DEPARTMENT, OFFICER LYNCH AND ROCC OCCURRED AT 1500. THE ROCKLEDGE POLICE DEPARTMENT SECURED THE SCENE, WHICH WAS THE PARKING LOT AT 435 GUS HIPP BLVD ROCKLEDGE, FL. I ARRIVED ON SCENE AT APPROXIMATELY 1600. THE PILOT MET ME AT THE AIRCRAFT AND PRESENTED HIS PILOT?S CERTIFICATE, MEDICAL CERTIFICATE AND DRIVER?S LICENSE. ALL OF WHICH WERE IN ORDER. I INSPECTED THE AIRCRAFT EXTERIOR, INTERIOR AND ENGINE AND FOUND NO OBVIOUS INDICATORS. I INTERVIEWED THE PILOT AND FOUND A FEW ITEMS OF INTEREST:

?	THE AIRCRAFT HAD SAT WITH 2 GALLONS OF FUEL, AUTO GAS WITH ETHANOL 10%, SINCE AROUND CHRISTMAS 2015, APPROXIMATELY 75 ? 90 DAYS,

?	THE PILOT MITIGATED THAT RISK BY ADDING FUEL ADDITIVE AND 7 GALLONS OF NEW FUEL WITH ETHANOL 10%,

?	I REQUESTED A FUEL SAMPLE BUT THE PILOT REPORTED THAT THIS AIRCRAFT DOES NOT HAVE AN INTEGRATED MEANS FOR SAMPLING,

?	THE FUEL LINES ARE BLACK RUBBER AND ABOUT 9 YEARS OLD, PER THE DATE STAMP, AND BEGINNING TO DECAY, PHOTO SHOWS CRACKING ON THE END OF THE LINE.

THE PILOT RAN THE ENGINE AFTER THE INCIDENT AND AFTER MY INSPECTION OF THE AIRCRAFT. THE AIRCRAFT STARTED FAIRLY EASILY AND RAN SMOOTH; HOWEVER IT FAILED THE INITIAL RUN-UP CHECKS AND INDICATED THE AIRCRAFT MAY HAVE A PARTIALLY CLOGGED FUEL LINE OR BOWL AT OR NEAR THE ENGINE.

MARCH 30,2016 EXAMINATION OF THE CARBURATOR FUEL BOWL REVEALED RUBBER FUEL LINE DEBIS AND YELLOWED FUEL.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
