As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JUNE 20TH, 2016 AT 14:32, THE PILOT AND TWO PASSENGERS DEPARTED RUNWAY 18 FULL LENGTH AT N30. THE PILOT STATED, "WHEN I REACHED 65 KTS I ROTATED OFF THE GROUND USING ABOUT 1/3 OF THE RUNWAY. WHEN I WAS ABOUT 30-40 FEET IN THE AIR I FELT THE AIRCRAFT WAS NOT CLIMBING AND I WAS NOT GOING TO MAKE IT OVER THE TREES AT THE END OF THE RUNWAY." THE PILOT ELECTED TO ABORT THE TAKE OFF. THE AIRCRAFT SUBSEQUENTLY DEPARTED THE END OF THE RUNWAY AND TRAVELED DOWN AN EMBANKMENT CAUSING SUBSTANCIAL DAMAGE TO THE AIRCRAFT.
