## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

SHORTLY AFTER TAKE OFF FROM RUN WAY 31 AT THE DSM AIRPORT ON A HEADING OF 330 THE PILOT REPORTED SUDDEN ENGINE POWER LOSS. IMMEDIATELY AFTER THE RADIO CALL TO THE TOWER AND AT AN ALTITUDE BETWEEN 800-900 FT AGL THE PILOT DEPLOYED THE PARACHUTE SYSTEM. THE AIRCRAFT DESCENDED THROUGH POWER LINES AND CAME TO REST IN THE INTERSECTION OF 56TH AND PARK AVE NW OF THE DES MOINES AIRPORT. ALL PERSONNEL ON BOARD THE AIRCRAFT EXITED BEFORE IT WAS CONSUMED BY FIRE. THE AIRCRAFT WAS NEARLY INTACT FIREWALL FORWARD WHICH ALLOWED FOR AN EXAMINATION OF THE ENGINE AT THE ACCIDENT SITE. A DISCONNECTED INTAKE TUBE COUPLER WAS DISCOVERED ON THE #1 CYLINDER ALONG WITH A FRACTURED INTAKE TUBE FLANGE ON THE #3 CYLINDER. FOLLOW ON INSPECTIONS WERE PERFORMED BY CIRRUS AND CONTINENTAL. IT WAS DETERMINED THAT THE LOSS OF ENGINE POWER WAS THE RESULT OF THE DISCONNECTED COUPLER AND FRACTURED INTAKE TUBE FLANGE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN16LA320](https://openairdocs.org/ntsb/CEN16LA320), Aug 13, 2016: Cirrus Design SR20 (N314BF), Des Moines, IA, destroyed. Probable cause: The two separations in the induction system due to improper installation and/or maintenance, which resulted in a partial loss of engine power during initial climb.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
