## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AN AGUSTA A109S AIR EVACUATION HELICOPTER WAS ENROUTE FROM BRAINERD LAKES REGIONAL AIRPORT (KBRD) TO CHANDLER AIRPORT, ALEXANDRIA, MN (KAXN) FOR A PATIENT PICKUP. DURING THE GPS APPROACH TO RUNWAY 22 AT KAXN, PILOT EXECUTED THE MISSED APPROACH DUE TO LOW VISIBILITY. THE AIRCRAFT INITIALLY PROCEEDED TO CLIMB AND MAKE A SLIGHT RIGHT TURN. AT APPROXIMATELY 270 DEGREES THROUGH THE TURN, THE AIRCRAFT WAS IN A STEEP DESCENT AND HIGH BANK TURN. DURING THE DESCENT, THE AIRCRAFT IMPACTED THE TOP OF SOME TREES BEFORE CONTINUNING TO THE GROUND, BOUNCING, AND HITTING NUMEROUS OTHER TREES BEFORE COMING TO REST IN A WOODED AREA ON ITS SIDE. TWO CREW MEMBERS WERE EJECTED FROM THE AIRCRAFT WHILE ONE WAS STUCK IN THE FRONT SEAT. ALL CREW MEMBERS WERE EVACUATED FOR MEDICAL TREATMENT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN16FA372](https://openairdocs.org/ntsb/CEN16FA372), Sep 17, 2016: Agusta A109 (N91NM), Alexandria, MN, destroyed. Probable cause: The pilot's excessive cyclic input during a missed approach maneuver in night instrument meteorological conditions, which resulted in a loss of control and spiraling descent into terrain.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
