## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

SIGNIFICANT ICING EVENT DEPARTING JUNEAU, WI LANDING AT WAUKESHA, WI.

PILOT RECEIVED WEATHER INFO VIA INTERNET FOR FREEZING DRIZZLE. SHE PRE-FLIGHTED THE AIRCRAFT IN THE HANGAR AND RECEIVED CLEARANCE FROM CELL PHONE FROM WITHIN THE HANGAR. PILOT FAILED TO CHECK THE DE-ICE/ ANTI-ICE EQUIPMENT DURING THE PREFLIGHT AND RUNUP CHECKLIST. SHE ENTERED AIRCRAFT WHILE STILL IN HANGAR AND LINE SERVICE MOVED TO RAMP. ENGINES STARTED AND IMMEDIATELY TAXIED TO CLOSEST RUNWAY FOR DEPARTURE WHILE PERFORMING A MODIFIED PRE-TAKE OFF CHECKLIST. DURING ACCENT, CRUISE AND DECENT, SHE NOTED ACCUMULATING ICE. ONCE ON APPROACH, SHE NOTED THE WINDSCREEN WAS COVERED WITH ICE AND A MISSED APPROACH WAS PERFORMED. DURING THE SEQUENCE, SHE COULD NOT DETERMINE IF THE ICE PLATE WAS WORKING AND ALSO COULD NOT DETERMINE WHICH SWITCH ACTIVATED THE ICE PLATE. SHE ASKED MKE APPROACH FOR A FREQUENCY CHANGE AND CALLED THE JUNEAU FBO FOR SOMEONE TO HELP HER IDENTIFY THE CORRECT SWITCH TO ACTIVATE THE ICE PLATE. A LINE SERVICE INDIVIDUAL MONITORING THE RADIO ANSWERED AND SHE ASKED HIM TO CALL HER FLIGHT INSTRUCTOR TO ASK WHERE THE SWITCH WAS. THE FLIGHT INSTRUCTOR WAS CONTACTED AND THE INFORMATION WAS RELAYED TO HER. SHE LOCATED THE SWITCH, ACTIVATED IT AND THE ICE PLATE BEGAN TO MELT THE ICE INTO AN 8 INCH CLEAR CIRCLE. SHE THEN CONTACTED MKE APPROACH AND BEGAN ANOTHER ATTEMPT FOR LANDING. SHE CONFIGURED THE AIRCRAFT WITH 15 DEGREES OF FLAPS TO SLOW ENOUGH FOR SAFE GEAR SPEED AND SELECTED GEAR DOWN. AD98-04-28 STATES NOT TO EXTEND FLAP UNTIL AIRFRAME IS CLEAR OF ICE. SHE NOTICED THAT SHE WAS A LITTLE LOW ON THE GLIDE SLOPE AND LOOKED OUT THE WINDSCREEN AN SAW ALL RED ON THE VASI. SHE ADVANCED THE POWER A BIT WHICH WAS AT 25" OF MANIFOLD PRESSURE AND AT WHICH POINT THE AIRCRAFT TOUCHED DOWN SHORT OF THE RUNWAY AND SKIDDED ONTO THE RUNWAY COMING TO REST ON THE SIDE OF THE RUNWAY.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN17CA114](https://openairdocs.org/ntsb/CEN17CA114), Feb 25, 2017: Cessna 340A (N255BC), Waukesha, WI, substantial damage. Probable cause: The pilot’s decision to fly in in conditions conducive to structural icing and her subsequent failure to maintain airplane control during the instrument approach.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
