## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON 05/05/2017 AROUND 0830 I(ASI JOSE F.ZAYAS) RECEIVED A CALL FROM MR. EDGAR SIERRA (ARECIBO AIRPORT MANAGER) REPORTING THAT AN EXPERIMENTAL AIRCRAFT HAD CRASH AT THE ARECIBO AIRPORT, REGISTRATION NUMBER N228KM. HE STATED THAT THERE WERE NO INJURIES TO THE PILOT. AVIATION SAFETY INSPECTOR RAFAEL GONZALEZ AND I DEPARTED IMMEDIATELY FOR THE AIRPORT. UPON ARRIVAL WE WERE ESCORTED TO THE CRASH SITE BY MR. SIERRA. WE IMMEDIATELY NOTICED SOME TREE TOPS WITH SOME DAMAGE ON THE NORTH SIDE OF RUNWAY 8. ABOUT 200 FEET FROM THE RUNWAY ON A GRASSY AREA WAS THE AIRCRAFT LAYING UPSIDE DOWN. I STARTED TAKING PICTURES OF THE SCENE WHICH WERE PROVIDED TO THE NTSB. SHORTLY AFTER OUR ARRIVAL, THE PILOT ^PRIVACY DATA OMITTED^ ARRIVED AT THE SCENE. WE STARTED ASKING HIM ABOUT THE EVENTS AND HE PROCEEDED TO EXPLAIN WHAT HAD HAPPENED. ON HIS STATEMENT HE STATED THAT AFTER TAKING OFF FROM RUNWAY 8 AND STAYING ABOUT 40 FEET AGL SUDDENLY FELT LIKE HE HIT SOMETHING IN THE LEFT WING AND IMMEDIATELY FELT IN AN INVERTED POSITION BEFORE IMPACTING THE GROUND.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB GAA17CA261](https://openairdocs.org/ntsb/GAA17CA261), May 5, 2017: Quicksilver Manufacturing SPORT2S R582 (N228KM), Arecibo, PR, substantial damage. Probable cause: The pilot’s failure to see and avoid trees during low-altitude flight.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
