## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

UPS FLIGHT 421, TAIL NUMBER N304UP, A BOEING 767, LANDED ON RUNWAY 17R AT APPROXIMATELY 0503 Z, 0103 EDT, AND TAXIED TO THE RAMP WITHOUT INCIDENT. IT WAS LATER REPORTED BY UPS THAT THE AIRCRAFT WAS DAMAGED BY FOREIGN OBJECT DEBRIS (FOD) ON THE RUNWAY. THE RUNWAY WAS CLOSED AT 0521 Z, 0121 EDT, DUE TO THE REPORTED FOD. THE FOD THAT WAS FOUND WAS A PIECE OF RUNWAY REPAIR THAT HAD LIFTED UPON TOUCHDOWN AND CAUSED DAMAGE TO THE AIRCRAFT. UPS REPORTED DAMAGE TO THE LEFT-HAND INBOARD AFT FLAP, TWO MAIN LANDING GEAR CONDUITS, AND A WHEEL ASSEMBLY (PICTURES ARE ATTACHED FOLLOWING THIS NARRATIVE). THE AIRCRAFT WAS TAKEN OUR TO SERVICE FOR MAINTENANCE TO BE PERFORMED. ALL PARTS MENTIONED ABOVE REQUIRED REMOVAL AND REPLACEMENT TO RETURN THE AIRCRAFT TO AN AIRWORTHY CONDITION. A PIECE OF THE RUNWAY REPAIR WAS FOUND ON THE LEFT MLG TRUCK MEASURING APPROXIMATELY 4 X 2 X 6 INCHES AND IS ALSO SHOWN IN THE ATTACHED PICTURES.

THE LOUISVILLE REGIONAL AIRPORT AUTHORITY (LRAA) STATED THAT THE RUNWAY IS CHECKED FOR FOD TWICE A DAY; ONCE IN THE MORNING BETWEEN 0630 AND 0900, AND ONCE AT DUSK. THERE WERE NO OTHER REPORTS OF FOD DAMAGE ON RUNWAY 17R THAT NIGHT. THE LRAA HAD MAINTENANCE PERSONNEL LOCATE AND REPAIR THE HOLE IN THE RUNWAY AFTER IT WAS TAKEN OUT OF SERVICE. THE PATCH WAS LOCATED ON RWY 17R BETWEEN B5 & B4, RIGHT OF CENTERLINE. ACCORDING TO LRAA MAINTENANCE RECORDS, THAT SPECIFIC PATCH OCCURRED ON:

-9/5/2016 (COLD PATCH) (I.E. RAPID REPAIR)

-9/12/2016 (DELCRETE PATCH) (I.E., PERMANENT PATCH REPLACED COLD PATCH/TAKES AT LEAST 6 HOURS TO CURE.)

THE LRAA CONFIRMS THERE WAS NO CONCRETE WORK DONE ON THE AIRPORT ON 06/28/17. ON TUESDAY 06/27/17, THEY HAD 17R/35L CLOSED FOR RUBBER REMOVAL ONLY.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
