- [NTSB GAA17CA383](https://openairdocs.org/ntsb/GAA17CA383), Jul 4, 2017: Robinson Helicopter R44 (N3025J), Easton, PA, substantial damage. Probable cause: The pilot’s distraction during liftoff due to the tail rotor chip warning light illuminating, which resulted in the helicopter drifting right, the skid touching down, and a dynamic roll-over.

Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong.
