## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE AMPHIBIOUS ACFT FLIPPED OVER DURING A LDG ON LAKE ASHBY WITH THE LDG GEAR EXTENDED. THE PIC SUSTAINED MINOR INJURIES WHILE THE PAX SUSTAINED SERIOUS INJURIES. THE ACFT WAS SUBSTANTIALLY DAMAGED, WITH THE MAJORITY OF DAMAGE SUSTAINED BY THE FWD HULL AND COCKPIT AREA.

THE PIC REPORTED THAT HE JUST FORGOT TO RAISE THE LDG GEAR BEFORE LDG ON THE LAKE. HE NOTED THAT HE RECENTLY PURCHASED THIS AMPHIBIOUS ACFT, AND THAT THIS WAS HIS FIRST FLT IN THE ACFT AFTER COMPLETING DUAL INITIAL TRAINING IN IT WITH A CFI AND OBTAINING AN EDORSEMENT.

WHEN ASKED ABOUT SEAT BELT USAGE, THE PIC REPORTED THAT HE AND HIS PAX WERE ONLY WEARING LAP BELTS AND WERE NOT USING THE SHOULDER HARNESSES INSTALLED IN THE ACFT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA17CA236](https://openairdocs.org/ntsb/ERA17CA236), Jul 8, 2017: Progressive Aerodyne Searey LSA (N796SR), Lake Ashby, FL, substantial damage. Probable cause: The pilot's incorrect configuration of the landing gear for a water landing in an amphibious airplane, which resulted in a nose-over.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
