## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT HAD BEEN FLYING FOR ABOUT 6 WEEKS STRAIGHT DURING THE FISHING SEASON. PILOT HAD TAKEN A FEW DAYS OFF TO REST AND WAS GOING FLYING THAT DAY WITH 1 PASSENGER ON A PERSONAL, PART 91 FLIGHT. PILOT STARTED TAKEOFF ROLL AND DID NOT UTILIZE FLAPS DURING TAKEOFF. PILOT WAS UNABLE TO BECOME AIRBORNE AND CRASHED INTO THE LAKE BANK ON SHANNON'S POND, DILLINGHAM. PILOT STATED HE WAS WELL RESTED, HYDRATED AND HAD NO PHYSICAL ISSUES PERTAINING TO ACCIDENT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ANC17CA038](https://openairdocs.org/ntsb/ANC17CA038), Jul 21, 2017: Dehavilland DHC 2 (N364RA), Dilllingham, AK, substantial damage. Probable cause: The pilot's failure to set the wing flaps to the "takeoff" position, which resulted in a longer-than-normal takeoff run and collision with terrain.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
