## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

TANDEM PAIR (INSTRUCTOR AND STUDENT) EXITED THE AIRCRAFT. THE DROGUE BRIDLE WAS RELEASED BY THE INSTRUCTOR, HOWEVER, IT WRAPPED AROUND THE INSTRUCTORS FOOT. THE INSTRUCTOR CUT-AWAY THE MAIN PARACHUTE, AND WENT STRAIGHT TO RESERVE DEPLOYMENT. WHEN THE RESERVE PARACHUTE DEPLOYED IT WRAPPED AROUND THE DROGUE BRIDLE CAUSING THE RESERVE PARACHUTE LINES TO TANGLE. THE RESERVE PARACHUTE NEVER DEPLOYED FULLY, AND THE TANDEM PAIR IMPACTED THE GROUND IN A FIELD TO THE SOUTHWEST OF THE NEPHI, UT AIRPORT.

BRIEF EXPLANATION OF ISSUES INVOLVED: WHEN THE DROGUE WAS RELEASED BY THE INSTRUCTOR, IT ENDED UP GETTING CAUGHT ON THE INSTRUCTORS FOOT. WHEN THE RESERVE PARACHUTE DEPLOYED, THE LINES WRAPPED AROUND THE DROGUE BRIDLE. THE AUTOMATIC ACTIVATION DEVICE (AAD) DID FIRE. THE RESERVE PARACHUTE DID NOT DEPLOY FULLY, AND DID NOT GENERATE ENOUGH LIFT TO PREVENT HARD IMPACT WITH THE GROUND.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
